First agree what to observe and when to seek help with a professional. A diary preserves facts but cannot replace those instructions or monitor treatment adherence.
A worked example
Worked example, using a fictional request. A professional asks for a particular observation morning and evening. The morning is recorded; the evening is missed. A medicine schedule exists, but actual use is unconfirmed. The day therefore contains one observation, one gap and a planned schedule, not “two normal observations and treatment completed”. The person explicitly identifies the unknowns in text. The app cannot fill gaps or infer an action from a notification. If their condition worsens, they should not wait for a complete series before seeking help.
What to do
- 01
Keep agreed observations separate from your assumptions.
- 02
Save actual timing and descriptions of changes.
- 03
Distinguish a prescribed schedule from an action actually completed.
- 04
Follow medical advice if your condition worsens rather than waiting for another entry.
Common mistakes
- Measuring everything too often due to anxiety.
- Change the treatment yourself according to the note.
- Delay contacting your doctor until you complete the report.
Plans and completed actions are different facts
A schedule describes an intention or prescription, not evidence that an action happened. If actual medication use matters to a discussion, agree how to record it separately; a delivered alert is not confirmation. Keep the known observation time and identify retrospective entries. Do not expand monitoring unnecessarily simply because the app has multiple sections. During review, note gaps, input errors and changing circumstances. This format clarifies the history but does not assess treatment effectiveness, select doses or establish how long waiting for care would be safe.
How to check the result
- Observations match the agreed question.
- Gaps remain visible without guessed values.
- The schedule is not described as confirmed medication use.
- Instructions for seeking help are not replaced by an app chart.
For a later discussion, use preparing for an appointment with a diary to select known facts and separate questions.
How this works in Vesometr
Some measurements have dedicated forms; other observations can be written as text. A medicine card or notification does not prove a dose was taken. Journal notes, food and the medicine cabinet are not exported in the BP PDF.
Sources and fact checking
App features were checked against Vesometr 1.2.2. External sources explain general guidance, rather than validating individual medical conclusions.
- Vesometr 1.2.2: official APK — Identifies the release; journal behavior and PDF limits were also checked against its source code.

Wellbeing and notes
Mood, a personal rating and free-text context for the selected day.
The article and appendix do not determine the cause of symptoms. If your illness is severe, unusual, or worsening, seek medical attention regardless of your diary entries.
