How to Use the Pain Scale in the Symptom Diary

The pain scale helps describe changes in the same sensation over time, but does not indicate cause or determine urgency on its own.

The short answer

If a professional provides a pain scale, follow its rules in the separately agreed form. Use a journal note for location, character, onset and effects on ordinary activities.

A worked example

Worked example, using a fictional request. A professional asks for a ten-point pain rating on a separate form. The person records “4 out of 10” there and writes in Vesometr: “right foot; onset around 17:00; interrupted my usual walk”. The outcome is a score with known rules plus a separate event description. Selecting the fourth emoji would not represent “4 out of 10”: it measures something different. The BP PDF will not include the journal note. Neither the number nor text enables the app to diagnose or recommend treatment.

What to do

  1. 01

Agree the required assessment format with the professional.

  2. 02

Describe location, character and timing in ordinary app text.

  3. 03

Keep numeric scales separately under their own rules, not as mood emojis.

  4. 04

Seek help for severe, unusual or worsening pain rather than waiting for more entries.

Common mistakes

  • Compare your scale with someone else's.
  • Describe only the figure without localization.
  • Delay seeking care just to collect more observations.

A number needs its context

Even the same score from one person may describe different sensations and circumstances. Add an understandable location, onset and effect on an ordinary activity without naming a diagnosis. If scales change, do not merge scores into one unexplained series. Do not use mood selection to measure pain intensity or wait to finish a diary when help is needed. Before a consultation, check that the professional receives both the agreed external form and the relevant description. Having records does not establish that waiting is safe or explain a disease.

How to check the result

  • The scale and rules are identified in the agreed external format.
  • Mood emojis do not stand in for pain ratings.
  • The description includes location and onset.
  • Sharing the free-text note is checked separately from the BP PDF.

How this works in Vesometr

Vesometr has no dedicated pain scale. The five emojis refer to mood. Free-text journal notes are not exported in the BP PDF; prepare a separate extract for sharing.

Sources and fact checking

App features were checked against Vesometr 1.2.2. External sources explain general guidance, rather than validating individual medical conclusions.

  • Vesometr 1.2.2: official APK — Identifies the release; journal behavior and PDF limits were also checked against its source code.
  • NIH: Pain — Supports describing location, duration, character and effects on activity; numeric ratings need clear definitions.

Wellbeing and notes

Mood, a personal rating and free-text context for the selected day.

Limits of self-observation

The article and appendix do not determine the cause of symptoms. If your illness is severe, unusual, or worsening, seek medical attention regardless of your diary entries.
